The Ready2Nurse Mission

During health events, we often rely on our loved ones to care for us, manage our affairs, and take on the emotional toll of long-term care. Caring for a loved one with chronic or debilitating health issues can have a significant impact on both the patient and the family. When health complications arise, caretaker burnout accelerates significantly without the assistance of a dedicated care team.

We are committed to providing compassionate and comprehensive care and conducting a unique assessment for each patient to create with a tailored nursing plan. The plan evolves constantly improving and changing to provide the utmost warmth and support and space to allow for as much independence and dignity as possible.

FOUNDER STORY

Ready2Nurse Home Health Agency was founded by Amy Wang FNP-C PA-C

Since 1994, I have helped thousands of patients and families cope with health crises in a variety of roles. I have visited patients both at home and in hospice settings to help provide physical assessments and plan care interventions, and these experiences have taught me a lot about planning quality home health care. I understand how important it is for patients and families to prepare for the inevitable, and processes like completing the 5 WISHES pamphlet and advanced directives are vital to help patients and their families be at peace.

I began planning Ready2Nurse when my dearest friend and mentor passed away on Easter in 2017. Before her passing, I was able to visit her home. This visit, similar to numerous experiences I've had visiting patients in the past, showed me firsthand the inadequacy of many home care and health assessment tools, among other issues. Over the years, my own frustration at seeing the lack of communication between healthcare providers and home health aides have made me committed to ensuring that patients and families do not feel isolated or helpless in times of crises.

I have been in the patient care business for more than 25 years, as a licensed Family Nurse Practitioner and certified Physician Assistant for 13 years and a Registered Nurse for 12 years before that. I received my medical training at Stanford School of Medicine for both my FNP and PA, and I worked in the Stanford University Medical Hospital before moving to a private family practice in San Jose. I also am an International Board Certified Lactation Consultant. I am fully dedicated to utilizing my clinical experience to improve one family health crisis at a time!
